Enclosed you will find patient history forms, our financial policy and a map to help you locate the practice.  Please complete the forms and bring them, along with your insurance card and a concise list of your current medications, to your appointment. 

Your appointment has been scheduled especially for you.  When you need to change or cancel an appointment, please notify us 24 hours in advance.  We value your time and make every effort to see you in a timely fashion.  Please plan on arriving fifteen minutes prior to your scheduled appointment.

All of our patients are required to sign an arbitration form at their first visit.  This is an office policy and exceptions are not made.

Our Office Manager Monica and Medical Assistant Cassie are happy to answer any questions prior to and during your visit.  We all look forward to meeting you.

Dr. Cunningham graduated from the Philadelphia College of Osteopathic Medicine.  She completed her internship and residency at the National Naval Medical Center in Bethesda, Maryland.  Dr. Cunningham served the United States Navy on active duty at the Naval Medical Center San Diego.  She is a Fellow of the American College of Obstetrics and Gynecology and is the San Diego District Vice-Chair.  Dr. Cunningham is board certified by the American Board of Obstetrics and Gynecology and is a member of the Active Attending Staff at Scripps Mercy and Sharp Mary Birch Hospitals.

Directions:

From North County:

Interstate 5 South to Washington Street exit.  Turn left onto Washington Street.  Turn left onto Fourth Avenue.  At the stop sign turn right onto Fifth Avenue.  Go around the curve at Scripps Mercy Hospital and our parking lot will be after the hospital ER parking lot.  It is the ACE parking lot adjacent to our building.

From East County:

Interstate 8 West to Highway 163 South.  Take the Washington Street West exit.  Turn right onto Fifth Avenue.  Our ACE parking lot is the first right and adjacent to our building.

From South County:

Interstate 805 North.  Take El Cajon Blvd West exit.  El Cajon Blvd changes name to Normal St.  Go west on Washington St.  Turn right onto Fifth Avenue.  Our ACE parking lot is the first right and adjacent to our building.
